Online Workshops is an online training initiative aimed at health professionals who require updates in clinical subjects. It has been developed by Professional Development International to provide an online cervical smear training update. In particular, it saves time and money.

Electronic Prior notification Lists
Electronic Prior notification Lists (e PNLs) have been introduced in Cornwall. Ten practices have signed up to it and are very positive about the new system and prefer this system to receiving paper copies. The PCSA would encourage practices to change to this system and are asking that people contact Tracy Cowling or Jayne Rogers at the PCSA on the e mail addresses below. They will need to sign up to the ‘Open Exeter’ system.
Other Benefits of signing up to the 'Open Exeter’ system:
- Pre populated HMR101 forms
By signing up the 'Open Exeter’ system practices now have the opportunity to print off pre populated HMR101 forms. This should save sample takers considerable time in practice.
- Non responders
Practices will soon be able to access the names of non responders through ' open Exeter' but this is in the pilot stage with NHAIS. This will mena practices will be able to use this infomration for completion of their cards and print off details.
- HPV vaccination uptake
The system has been recently updated to that information can be downloaded on the populations and uptake of HPV vaccination. Practices will be able to monitor the uptake / refusal or opting out of the smear and vaccination programmeStatistics will be used more for the NHSAIS system , QA, etc in the near future when the HPV programme has run for several years. Discussions are taking place between Child Health and PCSA as to who will populate the Open Exeter Website as practices are not currently funded to do this.

- Vault samples
The PCSA is no longer responsible for reminding practices of women who require a vault sample. Letters will be sent from colposcopy units and general practitioners. Letters will be sent to all practices from Dr Marian O’Donnell the Cancer Screening Commissioner to all practice manager and sample takers on the 24th /11/08 to this effect.

LBC Kits:
LBC kits are ordered via email address: This e-mail address is being protected from spambots. You need JavaScript enabled to view it. (surgery name and number of remaining kits should be stated in email). LBC kits must be ordered on regular, monthly basis. Orders must be placed well in advance to allow for delays in dispatch and transport.
Extra brushes:
Extra brushes can be ordered from Tricia Smith. Practice Nurse Lead.
Timing of samples:
Please ensure samples are taken midcycle, unless opportunistic. Contamination of the sample with blood and/or mucus increases the turnaround time for the report as well as the inadequacy rate. Please note, if the patient is using oestrogen cream, cease treatment about 5 days prior to sample taking to avoid contamination. Avoid use of lubricants. Please use warm water only as the use of lubricants is contributing to the reprocessing rates
Endocervical brushes:
Please note, if an endocervical brush is required, it must be used in conjunction with a LBC broom.

Updated January 2011. Please ensure samples are transported to the laboratory without delays to ensure compliance with the Cancer Reform Strategy, ie that 50% of women receive their results within 7 days, 95% within 14 days.

Patients with no cervix will be ceased from recall and any subsequent vault tests will not be actioned by the recall agencies. This means that a result letter will not be generated and so the sample taker must inform the patient of the result.
CHANGES TO CERVICAL CYTOLOGY REPORTING
Applicable to all Cervical LBC samples taken on or after 1st Feb 2010
Changes agreed by the Local Plymouth Cervical Screening Group and Cancer Services -
------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------Please circulate to all staff this may concern and amend your policies accordingly.
1. Reports issued as inadequate/unsatisfactory
In line with new national guidance all cervical LBC samples taken on or after 1st February 2010 and reported as inadequate should be repeated after 3 months replacing the current recommendation of 2 months.
To ensure consistency of communication both the cytology report sent to the sample taker and the result letter issued to the patient by the recall agency will recommend a 3 month recall interval.
Please note: repeat samples taken prematurely may affect the cellular yield of the sample. Midcycle samples are essential. In cases where the use of topical oestrogen is recommended, it is advisable to initiate the treatment in the second half of the recall interval and to cease treatment 5-7days before the sample is due to ensure optimal sample consistency.
2. Reports issued as ?invasive carcinoma or ?glandular neoplasia
All cervical LBC samples taken on or after 1st February 2010 reported as ?invasive carcinoma (result code 5) or ?glandular neoplasia (result code 6) will be treated as Direct Referrals.
It will therefore no longer be the responsibility of the sample taker to refer these patients. As with all other referrals to the Colposcopy Clinic, referral will be undertaken by the Cytology Laboratory. The reporting pathologist will continue to inform the patient's GP of the result by telephone and it remains the GP's responsibility to communicate the result to the patient as soon as possible.

Invitation letters
All screening invitation letters will now be sent from a central site. So instead of practices sending invitations to patients the PCSS will send them.

Prior Notification Lists
Prior notification list (PNL) will be sent from the PCSS ( Primary Care Support Services) to practices electronically instead of by hard paper copy.
14 day turnaround times
The government target for the turnaround of the time the sample is taken to the time the woman receives her result will be no longer than 14 days. We aim to achieve this across Devon as soon as possible. To help us to achieve this please ensure that samples are sent to the laboratory without delay.
Inappropriate removal of women from the screening prgramme
Please carefully observe your ‘informed dissenters‘ list to ensure that all those women who have been removed from the screening programme are appropriately entered.
Results letters
Women will now receive their results directly from the PCSS . A copy of the results will be sent to general practices.

HPV Testing
The Bristol lab is one of those involved in the Sentinel sites project to look at feasibility of implementing HPV testing into the routine lab setting. Currently samples showing low grade abnormalities on cytology are tested for high risk HPV types. If positive, they are referred to colposcopy, if negative they are returned to routine recall. We are also using HPV testing as a test of cure for women who have been treated for high grade CIN. If cytology and HPV test are negative, these women can be put on 3 year recall instead of having annual follow up samples.

We have similar problems to other areas regarding reprocessing rates and would ask all sample takers to avoid the use of lubricants and avoid including plugs of mucus that block the filters.
